Register With Us
New patients are most welcome and are registered at the discretion of the GP.
We do not take requests over the phone/email but ask you to call into the surgery for a registration form. When completed we ask for two forms of ID one with a photo and the other with recent proof of address. This can take up to two weeks to process. (Please make sure you have enough medication from your previous surgery for a month)
We do ask that anyone over 18 comes in with their own registration form and ID proof.
When you are registered on the system we will inform you and then invite you to attend a new patient health check with our health care assistant.

Named Accountable GP
Once registered at Field Street Surgery, you will have a named doctor who has overall responsibility for your care and support. Your registered GP is also your named accountable and allocated GP.
They will be responsible for the provision of your healthcare - if you choose to see another doctor at the surgery, you are entirely free to go on doing so exactly as before. If you are unsure of who your named doctor is, please ask at reception.
New patients are allocated a Doctor on Registration.
